Attention Parents of upcoming 12th grade students:
The Pennsylvania Department of Health has added an additional immunization to the state school requirements. A second dose of the meningococcal vaccine will now be required for entry into 12th grade. This is a mandatory immunization and you must present written proof that your child has received this second dose of the meningococcal vaccine before the first day of the 17/18 school year. Please contact your doctor to see if your child has already received this dose or to schedule an appointment to receive it and send a copy of your child's immunization record from the doctor's office into school as soon as possible. This immunization can be given at age 16 or older. If you have any questions please call the school nurses' office.
Attention Parents of upcoming 11th grade students:
The Pennsylvania Department of Health requires that all eleventh grade students have a current Physical Exam Report on file in their School Health Record.  We can accept your child’s Physical Exam Report if it was completed in either grade 10 or grade 11. Please return, fax or email your child’s Physical Exam Report to the school Nurse, as soon as possible.  A physical exam form can be downloaded from the YSSD Website or you can submit an exam form from your Health Care Provider. A copy of your child's drivers permit or license is also acceptable proof that they have had a physical exam.Students unable to obtain a Physical Exam through their own Health Care Provider will be scheduled for a Physical Exam with the School Physician, in order to meet the state requirement.

Thank you for your immediate attention to this very important matter.  Please contact the school nurses' office if you have any questions.  To reach the nurse's office, please call the high school at (717) 885-1270 and ask for the nurse.
