Required Documents for Enrollment

What to Collect Before Starting an Enrollment Application

Most or all of these documents will be required in order for the student’s application to be considered. Additional documents may be required depending on your situation. It is recommended that you scan and save each document electronically prior to starting the application.

1. Proof of Residency

    • Two documents will be required as proof of residency, one from each of the following sections:
      • SECTION ONE
        • A current lease listing the names of the minor occupants, among other basic requirements
        • The most recent real estate tax bill if it’s in the name of at least one of the student’s biological parents or legal guardians with whom the student lives
        • The final, signed version of the Closing Disclosure or Settlement Statement if it’s in the name of at least one of the student’s biological parents or legal guardians with whom the student lives and a real estate tax bill has not yet been received
        • Completed residency affidavit if the biological parents or legal guardians with whom the student lives do not rent and do not own the home
      • SECTION TWO
          • Current PA driver's license or photo ID
          • Current PA vehicle registration
          • Current PA voter registration card
          • Current homeowner renter insurance paperwork
          • Current vehicle insurance paperwork
          • Current utility bill (gas, electric, water, sewer)
          • Copy of a current tax return
          • Current bank statement

2. Photo ID of the parent/guardian completing the application

    • If the name on the photo ID does not match the name on the child's birth certificate, please have a marriage certificate or other name change document available to upload.

3. Immunization records for the student(s)

    • IMPORTANT: Students being enrolled in kindergarten must turn five prior to September 1st. Students being enrolled in first grade must turn six prior to September 1st.

7. Optional items for application, but required by first day

  • The following items can be uploaded into the application. While they are not required in order to submit an application, you will need to submit them directly to the school building if they are not uploaded during the registration process.
    • Dental exam records (use the form located here)
    • Physical exam records (use the form located here)
    • Student health history (use the form located here)
    • Physician's order for prescribed medication (if applicable)
