Reminders About Required Health Documents and Immunizations

Dear York Suburban Families,
For the 2022-23 school year, we are asking parents and guardians to plan ahead regarding required health documents and immunizations. Being prepared allows our York Suburban School District nurses to best serve our students.
The following exams must be conducted for the listed incoming grades as required by PA Public School Code. The exams must have been completed after July 1, 2021. Printable forms can be found at this link.
Kindergarten: Private Physical Examination and Dental Screening
Grade 3: Dental Screening
Grade 6: Private Physical Examination
Grade 7: Dental Screening
Grade 11: Private Physical Examination*
*For Grade 11 only, this exam can happen anytime during the prior year. Sports physicals, as well as copies of drivers’ licenses, can be accepted.
Additionally, certain immunizations are also required for all children attending Pennsylvania schools. The immunization law in Pennsylvania requires that all students have requisite immunizations by the first day of school unless the child has a medical or religious/philosophical exemption. Students in all grades must have at least one dose of the vaccinations listed below.
- 4 doses of tetanus, diphtheria, and acellular pertussis
- 4 doses of polio
- 2 doses of measles, mumps, and rubella
- 3 doses of hepatitis B
- 2 doses of varicella (chickenpox) or evidence of immunity
If a child does not have all of the doses listed above and the next dose is not medically appropriate, the child must provide an alternative medical plan for obtaining the required immunizations. This plan must be received within the first five days of school or the student will risk exclusion.
Finally, the vaccines required for entrance into Grades 7 and 12 are listed below.
Grade 7: 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade AND 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade.
Grade 12: 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 12th grade. If one dose was given at 16 years of age or older, that shall count as the 12th-grade dose.
